The causes of everything that goes right, as well as everything that goes wrong, can be traced all the way back to the beginning of time. This is what makes "root cause analysis" difficult, if not impossible.
The root causes of everything that goes wrong can be traced to human beings. We either do things we should not have done or don't do things we should have done. However, instead of blaming one another for our problems, each of us should be looking at ourselves, asking "What is about the way I am that contributes to my problems, and what am I willing to do about it?“
